Annual Health Assessments for over 75 year patients

Our Practice Nurse, Julie Grant is now attending to the Annual Health Assessments for all patients over 75 years of age. These assessments are usually best done in the home. Once completed, you will be required to attend the surgery for an appointment with your usual GP to finalise and discuss the assessment.

If you would prefer, the assessment could be completed in the surgery, upon request.

If you are 75 years or older and have not been contacted yet, please enquire at reception.

  • SAT: 8:30AM – 12:00 Noon

  • SUN/Public Holiday: Closed

Nurse

  • MON – WED: 9:00AM – 4:00PM

  • THU-FRI: 9:00AM – 5:00PM

After Hours

For after hours home visits, please contact:

The National Home Doctor Service on 13 74 25. They are available weeknights 7pm-6am, weekends from noon Saturday to 6.00am Monday and Public holidays from 7am-6am the following morning.
