Once completed, you will be required to attend the surgery for an appointment with your usual GP to finalise and discuss the assessment.
If you would prefer, the assessment could be completed in the surgery, upon request.
If you are 75 years or older and have not been contacted yet, please enquire at reception.
Our practice is now offering an added service of INR management for your convenience. If you require your INR to be checked, our nurses can do this for you, by an appointment only system.
